Population Served: Individuals of all ages recovering from illness, surgery, clients who are homeless, poorly housed, have mental health or cognitive impairment issues, physically challenged people and their caregivers.
Additional Information: An assessment is necessary for determining whether clients are eligible for CCAC services. CCAC will help you coordinate a plan that is responsive to you and your caregiver/family support system.
