Population Served: Individuals 60 years of age or older who: are experiencing a mental health issue; display significant changes in mood and behaviour; have difficulty managing safely due to changes in their mental function; are medically stable; reside in the community (including a long-term care facility or retirement residence).
Also available to care givers experiencing stress while caring for a relative and who would like information about local community services and long-term care facilities.
Area Served: Etobicoke, York, North York (west of Yonge St).
Additional Information: The professionals of West Park's Seniors Mental Health Service provide free in-home assessments and practical advice to help individuals and families deal with issues like difficult behaviour, stress, depression, confusion and dementia.
Family is encouraged to attend a comprehensive, 90-minute in-home assessment conducted by experienced mental health consultants (social worker and nursing backgrounds).
Consultation with psychiatrist to determine most appropriate intervention plan.
Clients and families are provided practical advice to help individuals and families manage difficult behaviour, stress, depression, confusion and Alzheimer's disease or related dementia.
Written recommendations for treatment shared with the family physician.
Information about community resources and/or long-term care facilities.
Education for families and professional caregivers regarding mental health issues.
