Population Served: Persons with memory or cognitive problems of any age. People suffering disturbances in cognitive function and behaviour due to brain dysfunction. Problems may include memory loss, speech and language disorders, as well as cognitive deficits associated with psychiatric disorders (e.g., depression).

Please note that a separate Memory Clinic referral form is available and may be obtained by contacting the Memory Clinic main number (416-864-5320).
The Memory Clinic offers comprehensive diagnostic assessment and follow-up of patients. Initial assessments are provided by a geriatric psychiatrist and/or a nurse clinician. Neuropsychological testing is available on a limited basis to help assist with diagnosis and treatment.
Services provided include assessment and diagnosis, treatment, follow-up and education. In addition, clients may have an opportunity to participate in clinical research.
Referrals are made to the CCAC where appropriate.
