| Cost: | There is not charge to the participants for the services of the Board. The Board is publicly funded and requests that all participants assist in keeping costs down |
| Population Served: | Hearings are available for individuals incapable of making decisions about personal assistance services, treatment decisions, or admission to a care facility. Individuals may apply on their own or with the assistance of someone else, i.e.. a member of your staff, a friend, a lawyer, rights advisor or a member of an advocacy group. |

| Additional Information: | The board has authority to hold hearings to deal with matters such as: Health Care Consent Act, Mental Health Act, Personal Health Information Protection Act, and Substitute Decisions Act.
