have health care needs that cannot be met with any combination of caregiving or community-based services in the home;
have health care needs that can be met in a long-term care facility.

Additional Information: Cedarhurst is designed to meet the needs of those with early-to-mid-stage AD. Activation program is designed for their unique needs and considered an important part of their care during their stay.

Short to mid-term stays can be arranged from 1 week to 3 months, depending on availability.

Unit description: Private rooms with ensuite bathroom, share common areas. Activation staff on-site daily to work with residents on individual basis. Suitable for families needing short term respite or wishing to tryout facility prior to longer term commitment.
Safety features: Small secure home where residents move about freely and safely both indoors and out - patio garden and balcony.Trained staff :resident ratio 1:6 day time, 1:12 overnight, can assist with necessary medications.

Full geriatric assessment
Medically stable
Able to feed himself/herself with minimal assistance
Can manage some self care (dressing and bathing) with the assistance of staff
Behaviour is appropriate for communal living and can be controlled with appropriate staff intervention
Not alcohol or tobacco dependent
Able to toilet self with reminders
Can follow instructions related to very simple, functional tasks
