Services - Supportive Housing

Supportive Housing is a Ministry of Health and Long-Term Care funded program that provides personal support services and essential homemaking services in selected buildings, seniors residences and congregate housing situations. The program is designed to assist people who need minimal to moderate care to live independently.
NOTE: These services may not be available to people with dementia. Please contact Housing Connections for further information at 416-981-6111.

Listed below are two residences that provide Supportive Housing for individuals in the early to mid stages of Alzheimer's disease or related dementias.
